Registration Process for New Hospital Units (ICU and Non-ICU)

There is no charge for hospital teams to participate in the two-year On the CUSP: Stop BSI in Florida's Hospitals collaborative, which begins in July 2010. Hospitals can register multiple units (with each unit registering individually). Teams should be prepared to participate fully in collaborative activities, including implementation of the CUSP and the CLABSI reduction protocols, teleconferences, in-person meetings, and data collection and submission.

To register please complete the collaborative registration process, which includes submitting the signed/completed documents listed below.

MHA Care Counts Registration Form:
Hospital units will be entering data into the MHA Care Counts database. Please complete the MHA Care Counts Registration Form so that your hospital/unit can be added into this Web tool before the immersion calls begin (submit a form for each unit).

Initial Foundational Immersion Call Series:
This collaborative will begin on July 8, 2010, with a series of weekly foundational immersion calls, listed below. These calls are designed to get your team comfortable with the concepts before full implementation of the CUSP/CLABSI effort. The call-in information will be sent to your team once units are registered. Immersion calls occur each Thursday at 4:00 pm EDT and will be recorded.
