Safety II Culture & Risk Reduction at a Large Medical Group

With IOM’s report, To Err is Human, CMS tried to apply safety lessons learned from the aviation industry with respect to reducing error. This approach of learning from defects can be rolled under the broad umbrella term of a Safety I model. Within the past 10 years, there is another model, safety II, that has immerged focusing on the conditions that drive success. We will discuss one medical group’s implementation of parts of Safety II framework. Through this lens, we will explore our efforts at risk reduction and how Safety II can be used to help mitigate liability payments.

Dr. Keenan Bora is an emergency medicine physician and a fellowship trained medical toxicologist. He currently works as the Vice President of Safety and Quality Registries for Envision Healthcare. He oversees the areas risk management, Patient Safety Organization membership, Envision’s QCDR and MIPS reporting. He loves cooking and eating good food, exercising, reading science fiction, watching movies with his family, and playing classical guitar.
